

Orchard Health Care membership fees reflect Dr. Kanner’s respect for the families in his practice and for the many multigenerational families for whom he has cared.
Annual Fees (calendar year 2012)
Individual: $2,900
Couple: $4,800
Child of member (age 16 to 29): $300
Young Adult (age 30 to 38): $1,450
Payment Methods
Annual membership fees may be prepaid by check or quarterly by credit card. Annual invoices only are sent to members in the fall. Payments are due in full by December 20 each year, or the first quarterly credit card payment is automatically charged at that time. Subsequent payments are charged March 20, June 20, and September 20.
Acceptable credit cards for automatic charges include MasterCard, Visa, and American Express.
Refunds
Prepaid Orchard Health Care membership fees are proportionately refundable, net of a modest termination fee, if a member moves or otherwise decides to leave the practice after an initial six-month commitment.

Financial matters relating to beginning a membership in Orchard Health Care or paying the annual fees are handled at the membership office.

Membership Fee for Member Benefits
Membership fees paid by the member to Orchard Health Care, LLC are solely for the member benefits that are provided, or arranged to be provided, by Orchard Health Care. The membership fee does not cover or pay for any of the clinical care provided by Dr. Kanner or his practice, Steven R. Kanner, MD, LLC, or any items or services covered by the member’s health plan.
